Your input in the process of developing the 2010-2015 CHS Strategic Plan is critically important and can be provided in a variety of ways including:
  1. Personal communications with the CHS Strategic Planning Committee members (including face-to-face, e-mail, telelphone, etc.)
  2. Attending the open forums.
  3. Submitting your feedback electronically using the form below. By doing so, your input will be added to the thread of responses on the feedback site.
